SPRAVATO and TRD
What is TRD and Why Is It Challenging to Treat?
When depression symptoms persist even after trying various medications, it is known as treatment-resistant depression (TRD). Approximately 4 million people suffer from TRD in the U.S. alone. Traditional antidepressants work for many, but an estimated 30% of patients continue to experience symptoms such as hopelessness, fatigue, and even suicidal thoughts despite multiple treatments. For these individuals, finding relief is a long, painful journey. However, SPRAVATO offers a new avenue of hope, especially for those facing the struggles of TRD.
What is SPRAVATO (Esketamine)?
SPRAVATO is a prescription medicine taken by nasal spray and is used to treat adults diagnosed with treatment-resistant depression. Approved by the FDA in 2019, it provides a revolutionary alternative to traditional antidepressants. Unlike standard antidepressants that primarily target serotonin and norepinephrine levels, SPRAVATO acts on a different system in the brain by targeting the NMDA receptor.
This unique mechanism allows SPRAVATO to work rapidly, often delivering relief within hours. For individuals who have suffered from depression for years, this rapid effect is a significant advancement compared to the weeks or months it can take for traditional antidepressants, such as SSRIs and SNRIs, to show results.
How SPRAVATO Works
SPRAVATO helps to restore balance in the brain’s neurotransmitter system, potentially reversing the long-term effects of depression. It achieves this by enhancing the brain’s adaptability, or neuroplasticity, providing a unique approach compared to standard SSRIs and SNRIs, which require extended periods to manifest their effects.
Who Can Benefit from SPRAVATO?
SPRAVATO is designed for adults diagnosed with treatment-resistant depression, particularly those who have failed to respond to at least two different antidepressants. In 2020, the FDA expanded its use to include individuals with major depressive disorder (MDD) who exhibit suicidal ideation.
Administration of SPRAVATO
SPRAVATO is administered as a nasal spray under strict medical supervision in certified healthcare facilities. A healthcare professional oversees the entire process to ensure safety. After administration, patients are required to remain in the clinic for at least two hours for observation. During this time, healthcare providers monitor for any immediate side effects such as dizziness, dissociation, or elevated blood pressure.
Treatment Phases
SPRAVATO treatment is divided into two distinct phases:
- Induction Phase: During the first four weeks, patients receive treatment twice a week.
- Maintenance Phase: Following the induction phase, the frequency of treatments decreases to once a week.
This structured approach ensures that patients can gradually adjust to the medication while experiencing sustained relief.
Effectiveness and Benefits of SPRAVATO
Clinical trials have shown that some patients experience significant improvements in depressive symptoms, often within hours or days after using SPRAVATO. This is a stark contrast to traditional antidepressants like SSRIs and SNRIs, which can take weeks to provide noticeable improvements. Additionally, long-term benefits include improved mood stability and functional recovery when SPRAVATO is used alongside oral antidepressants. The medication’s ability to target the glutamate system in the brain marks a critical advancement in depression treatment.
